The 1095-B forms are available now in BAM once the member has logged in.

The Affordable Care Act set reporting requirements related to minimum essential coverage. Individuals no longer have to show proof of health coverage, but insurers and employers are still required to report to the Internal Revenue Service (IRS) about the coverage they provide or offer.

• The IRS doesn’t require insurers to automatically mail 1095-B/C forms to members, so we don’t mail forms to members unless students request a copy.
• Those forms are available on Blue Access for Members (BAM). Members can also call the number on their ID card to request a physical copy or visit the web page - https://www.bcbsTX.com/insurance-basics/coverage-information/tax-penalty

STOP - If you are still attending the University of Houston and are eligible to enroll in the are eligible for UH Student Health Insurance Plan for the current coverage period - DO NOT ENROLL HERE.
Continuation of Coverage is only available to insured students who have graduated or are no longer eligible for coverage under the University of Houston Student Health Insurance Plan for the current coverage period. Students and their dependent(s) can purchase three (3) months of additional medical coverage. (Dental is not available.) Continuation of Coverage must be purchased within thirty-one (31) days of the current coverage termination date. No exceptions.

Covered students must have been insured for at least one (one) continuous semester before coverage terminated under the Prior and/or Current Plan.

Continuation of Coverage is in effect from the date coverage under UH Student Health Insurance Plan expires and continues until the end of the period for which the premium is paid. The total premium must be paid at the time of enrollment. There is no renewable option, and no refunds are available after you have selected the coverage.
